The licensee has not implemented the written policies and procedures for monitoring compliance and contraventions.

The parent handbook does not include information about the reasons for when refunds or credits of fees will be provided.

Not every employee who may be counted for the purposes of meeting the required ratios has a valid certification in first aid, including infant and child CPR, issued by a WSIB-approved training course.

The licensee has not stored items listed under ss.40(3) as non-medication in accordance with the instructions on the label and/or the container or package was not clearly labelled with the child's name and the name of the item.

The licensee has not kept up-to-date records of a child's previous history of communicable diseases, conditions requiring medical attention, immunization and/or written objection, as required.

Spaces for storage of medical supplies, cleaning materials, equipment, other poisonous or hazardous substances, heating equipment and/or electrical equipment in the child care centre, and the items kept in those spaces, are accessible to children.

The licensee has not kept play materials, equipment and furnishings in a safe condition and in a good state of repair.

The licensee has not implemented the written procedures approved by the local fire chief.

The licensee has not kept a written record of all fire drills, all tests of the fire alarm system and all tests of fire protection equipment for at least 12 months.

At least one individualized plan for a child with medical needs does not include a description of any medical devices and any instructions related to its use.
