The licensee has not stored items listed under ss.40(3) as non-medication in accordance with the instructions on the label and/or the container or package was not clearly labelled with the child's name and the name of the item.

The licensee has not provided a report to a program adviser within 24 hours of the licensee or supervisor becoming aware of the occurrence.

The licensee has not maintained a daily written record that includes a summary of any incident affecting the health, safety or well-being of any child.

The licensee has not implemented the anaphylactic policy.

The licensee has not implemented an individualized plan.

The licensee has not implemented the policies and procedures with respect to serious occurrences.

The licensee has not completed an accident report when a child is injured.

The licensee has not kept a daily attendance record showing the time of arrival and departure or the absence of each child.
