The licensee has not implemented the playground safety policy.

A plan has not been developed on how issues or problems identified in a playground inspection will be addressed.

A fixed play structure or surfacing in the outdoor play space does not meet the required standards.

The licensee has not sent a copy of the direction or order madeby the local medical officer of health or the local fire department immediately to a program adviser and/or has not notified a program adviser immediately of any enforcement action taken against the licensee in relation to the direction or order.

The licensee has not maintained a daily written record that includes a summary of any incident affecting the health, safety or well-being of any child.

The licensee has not posted a summary of a serious occurrence report and any action taken for at least 10 business days in a conspicuous place at the child care centre.

The licensee has not stored items listed under ss.40(3) as non-medication in accordance with the instructions on the label and/or the container or package was not clearly labelled with the child's name and the name of the item.

One or more offence declarations are not current and/or do not address the relevant time period.
