The licensee has not implemented the policies and procedures required under s. 65.

A fixed play structure or surfacing in the outdoor play space does not meet the required standards.

The licensee has not labelled food and/or drink supplied by a parent with the child's name.

The licensee has not posted a list in each cooking and serving area with names of the children and their allergies or food restrictions.

The licensee has not made a list available and accessible in any other area in which children may be present with names of the children and their allergies or food restrictions.

The parent handbook does not include information about the reasons for when refunds or credits of fees will be provided.

The licensee has not implemented the safe arrival and dismissal policy.

The licensee has not kept a daily attendance record showing the time of arrival and departure or the absence of each child.

Spaces for storage of medical supplies, cleaning materials, equipment, other poisonous or hazardous substances, heating equipment and/or electrical equipment in the child care centre, and the items kept in those spaces, are accessible to children.

The licensee has not made all drugs and medications inaccessible to children at all times.

The licensee has administered a non-medication item listed under ss.40(3) to a child without parent written authorization.
