An employee or volunteer of the licensee, or student who is on an educational placement with the licensee has engaged in a prohibited practice set out in subsection (1) with respect to a child receiving child care.

The licensee has not implemented the policies and procedures required under s. 65.

The licensee has not obtained a new vulnerable sector check from each person, on or before every fifth anniversary after the date of the most recent vulnerable sector check.

The licensee has not provided a report to a program adviser within 24 hours of the licensee or supervisor becoming aware of the occurrence.

The licensee has not implemented the policies and procedures with respect to serious occurrences.

Spaces for storage of medical supplies, cleaning materials, equipment, other poisonous or hazardous substances, heating equipment and/or electrical equipment in the child care centre, and the items kept in those spaces, are accessible to children.

The licensee has not ensured that a drug or medication is administered to a child only with written parental authorization including a schedule that sets out the times and amounts to be administered.

The licensee has not administered drugs and medications according to the instructions on the label and the parent's written authorization.

The licensee has not kept all drugs and medications in a locked container.

The licensee has not ensured that a drug or medication is administered to a child only from the original container clearly labelled with the child's name, the name of the drug or medication, the dosage, the date of purchase and expiration, if applicable, and instructions for storage and administration.

The licensee has not kept a daily attendance record showing the time of arrival and departure or the absence of each child.

The erection, use, alteration or renovation of the building, premises or playground has commenced without prior approval of the plans by a director.
