The licensee has not implemented the written procedures for the administration of medications.

The licensee has not ensured that a drug or medication is administered to a child only from the original container clearly labelled with the child's name, the name of the drug or medication, the dosage, the date of purchase and expiration, if applicable, and instructions for storage and administration.

The licensee has not maintained a daily written record that includes a summary of any incident affecting the health, safety or well-being of any child.

The licensee has not implemented the policies and procedures required under s. 65.

The licensee has not obtained a new offence declaration from each person in every calendar year as required.

The licensee has administered a non-medication item listed under ss.40(3) to a child without parent written authorization.

The supervisor does not have a valid certification in first aid, including infant and child CPR, issued by a WSIB-approved training course.
