An employee or volunteer of the licensee, or student who is on an educational placement with the licensee has engaged in a prohibited practice set out in subsection (1) with respect to a child receiving child care.

The licensee has not developed an up-to-date individualized support plan for each child with special needs.

The licensee has not implemented the policies and procedures with respect to serious occurrences.

The licensee has not implemented the approaches set out in its program statement.

Not every child has been supervised by an adult at all times.

The licensee has not implemented the playground safety policy.

Spaces for storage of medical supplies, cleaning materials, equipment, other poisonous or hazardous substances, heating equipment and/or electrical equipment in the child care centre, and the items kept in those spaces, are accessible to children.

The licensee has not kept up-to-date records of the date of admission of each child.

The licensee has not kept a daily attendance record of each licensed age group that includes the name of each child in the group and the times the child was in the group.

The licensee has not complied with the conditions on a licence.

The licensee has not employed a qualified supervisor.

The licensee has not maintained a daily written record that includes a summary of any incident affecting the health, safety or well-being of any child.

The licensee has not maintained the proportion of employees that must be qualified.
