The licensee has not kept a written record of all fire drills, all tests of the fire alarm system and all tests of fire protection equipment for at least 12 months.
The licensee has not maintained a daily written record that includes a summary of any incident affecting the health, safety or well-being of any child.
The licensee has not separated a child who appears to be ill and/or has not noted the symptoms of illness in the child's records.
One or more offence declarations are not current and/or do not address the relevant time period.
The licensee has not implemented the policies and procedures required under s. 65.
The licensee has not kept up-to-date records of a child's previous history of communicable diseases, conditions requiring medical attention, immunization and/or written objection, as required.
