The licensee has not obtained a new offence declaration from each person in every calendar year as required.

There is not an agreement between a home child care agency and a child care provider that provides for the agency's oversight of the provision of care.

There are no policies and procedures with respect to sleep.

Not every home child care provider and person ordinarily resident or present at a premises and every volunteer or student on educational placement has a health assessment and immunization as directed by the local medical officer of health as required.

A daily attendance record has not been kept showing the time of arrival and departure or the absence of each child.

A daily written record has not been maintained that includes a summary of any incident affecting the health, safety or well-being of any child.

The licensee has administered a non-medication item listed under ss.40(3) to a child without parent written authorization.

The licensee has not stored items listed under ss.40(3) as non-medication in accordance with the instructions on the label and/or the container or package was not clearly labelled with the child's name and the name of the item.
